Follow these simple steps to secure your Letter of Authorization (LOA) before your consultation.
1. Fill Out Form
Click the button below to submit your details and medical request online.
2. Wait for Processing
Please give us at least 24 hours to process your submitted request.
3. Claim Your LOA
Proceed to the HMO Help Desk on the day of your consultation to print your LOA.
Important Guidelines:
- The LOA must be completed at least two days before your availability date.
- Online approval operates Mondays through Saturdays, from 8:00 AM to 5:00 PM (Registration cut-off is at 4:00 PM).
- Registrations made on Sundays will be processed on Monday morning.
